Course Content - #3. Renal Disease and Failure. AN OVERVIEW OF RENAL DISEASERenal disease is a serious health issue, affecting an ever- increasing number of patients and resulting in a financial cost in the United States of more than $3. It demands the involvement of many other healthcare personnel, including mental health professionals. Renal disease generally falls into one of two categories: chronic kidney disease (CKD) or acute kidney injury (AKI) . However, CKD should not be viewed in simple mathematic terms. It is an ongoing process of renal injury that causes compensatory hyperfiltration in less- affected glomeruli, which eventually leads to the destruction of those glomeruli as well . Nutrition Guideline For Professional Reference Only Renal Applicable to: Nurses, Physicians and other Health Professionals January 2013 Page 5.5.1.4. Left untreated, this ongoing destruction results in a steady decline in renal function, which eventually affects not just the renal system but almost every organ system in the body . AKI may be classified either by the physiologic cause (i. In the United States, 9. GFR) < 1. 5 m. L/min/1. 7. 3. m. Therefore, the terms ESRD and kidney.
Although ESRD is generally associated with a GFR. L/min/1. 7. 3 m. 2, it is more importantly an. Medicare ESRD program . In some. cases, ESRD is also used rather loosely to refer to patients who are experiencing. CKD and are expected to begin dialysis in a matter of days or weeks. Practice guidelines published by the National Kidney Foundation (NKF) define and classify CKD . The staging system is used to predict associated complications (Table 1), with an emphasis on predicting the development of cardiovascular disease, the leading cause of death in ESRD patients and the cause of dramatically increased mortality rates in patients with even moderate kidney disease. Identifying and staging patients with impaired renal function is important in order to prevent or slow the onset of CKD and its complications. CLASSIFICATION OF CHRONIC KIDNEY DISEASEStage. GFR is the best measure of kidney function. Normal adult GFR is 1. L/min/1. 7. 3. m. Definitions of CKD are. CKD increases with age. Hypertension and diabetes mellitus are common diseases in the United States. Patients with diabetes have a much higher rate of hypertension than would be expected in. 130/80 mm Hg, with an optimal target of below 120/80 mm Hg, especially in patients with proteinuria or renal insufficiency.9,10 Learn about Capoten (Captopril) may treat, uses, dosage, side effects, drug interactions, warnings, patient labeling, reviews, and related medications. True determination of GFR is made by collecting urine for 2. Patient compliance with urine collection can prove quite difficult and 2. GFR (e. GFR), which determines GFR based on numerous biomarkers including serum creatinine. Learn about Diovan (Valsartan) may treat, uses, dosage, side effects, drug interactions, warnings, patient labeling, reviews, and related medications.Although exact statistics regarding the prevalence of mild- to- moderate renal failure are not available, the epidemiology of ESRD has been widely documented by the U. S. Renal Data System (USRDS), which collects statistics on all Medicare patients on dialysis. Since 1. 97. 4, Medicare coverage has been extended to virtually all patients on dialysis in the United States; therefore, these data are highly representative of the current dialysis population. According to the USRDS, the number of patients with ESRD in the United States was 6. The dialysis population increased to 4. New cases of ESRD with diabetes or hypertension listed as the primary. Other less common causes of ESRD include. Due to the prevalence of kidney. ESRD in the United States; however, these patients are reported within their original. New cases of diabetic ESRD are expectedly higher with increasing age in. Statistically, non- whites are four. The prevalence of ESRD per million remains much. African Americans than in other racial groups, at nearly 2 times higher than. Native Americans, 2. Asian Americans, and 4 times higher than whites. The cost of treating ESRD was $2. AKI in the Medicare population has reached an annual average of. AKI rates are significantly associated with aging and with black/African American race. The most common causes of AKI vary. The result is hyperfiltration in the surviving glomeruli (in an attempt by the body to increase GFR), which then causes ongoing glomerular stress, renal injury, and eventually glomerular destruction . This leads to a decrease in GFR and a continuance of the hyperperfusion destruction syndrome. Allowed to continue, the patient will inevitably develop ESRD and require dialysis or transplantation to avoid death from uremia. When treatment to reverse this process is initiated prior to the patient losing more than 4. After a patient has lost more than 5. The pathophysiology of AKI depends on the site of occurrence. Intrarenal or intrinsic. AKI, the result of damage to the renal parenchyma, may be a result of prolonged prerenal AKI. Postrenal. (obstructive) AKI results from physical obstruction of urine outflow and may be caused by. Unfortunately, contrast dye is nephrotoxic and exposes patients to the risk of AKI. It is estimated that cardiac catheterizations alone are responsible for 1. AKI annually . The overall incidence of contrast- induced nephropathy (CIN) is 5% to 3. Contrast dye nephropathy is the third. Risk factors include pre- existing renal disease (i. L or e. GFR less than 6. L/min/1. 7. 3 m. 2), age. CHF, and hypovolemia . Research indicates that furosemide and mannitol do not decrease the incidence of. CIN and in fact may increase the rate by dehydrating patients . Studies involving the prophylactic. N- acetylcysteine (NAC) have produced mixed results, possibly due. In one study, patients received NAC twice. To prevent one case of contrast- induced. NAC was 8. The traditional dose is. In a 2. 00. 4 study. L/kg for one hour prior to their procedure followed by 1 m. L/kg for 6 hours postprocedure. The study was ultimately halted early. Follow- up analysis showed that 8. CIN . Absolute data from the study showed that. A retrospective cohort study conducted. Mayo Clinic assessed the risk of CIN with the use of sodium bicarbonate, NAC, or a. Compared with no. CIN, while NAC. alone or in combination with sodium bicarbonate did not significantly affect the incidence of. CIN . As discussed, nephrogenic systemic fibrosis occurs almost exclusively in patients with advanced renal disease who receive gadolinium- based contrast dye. First identified in 1. California, this disease is systemic, can involve all major organs, and is potentially fatal. The exact mechanism by which gadolinium- based contrast causes nephrogenic systemic fibrosis is poorly understood. Patients can develop the disease at any time postexposure (including after several years), with most patients developing symptoms 2 to 8 weeks after exposure . Patients typically first present with distal extremity swelling followed by skin changes. While nephrogenic systemic fibrosis may not occur for years following exposure, after it develops patients may describe a progression of symptoms that occurs over days to months, with 5% of patients experiencing a rapid course . On clinical exam, the skin may appear . As gadolinium is frequently used in MRI or magnetic resonance angiogram (MRA) procedures, the patient may have a history of one of these procedures and having received an intravenous medication at the time of the study. Biopsy is necessary to confirm diagnosis. Treatment for nephrogenic systemic fibrosis is only symptom based, and there is no known cure. There is also no way of definitively preventing nephrogenic systemic fibrosis when administering gadolinium- based contrast medium to renal failure patients; postexposure dialysis does not appear to decrease the risk. Therefore, the FDA has issued several warnings regarding its administration to patients with renal disease . In 2. 01. 0, a . Most gadolinium- based products should not be used in patients with AKI or severe CKD, including gadopentetate dimeglumine (Magnevist), gadodiamide (Omniscan), and gadoversetamide (Opti. MARK). Clinicians should refer to manufacturers' instructions and FDA warnings prior to the use of gadolinium in all patients, especially those with known renal disease or patients with risk factors for acute and/or chronic kidney disease. PATIENT AND FAMILY EDUCATIONPatient education in renal failure is highly complex. CKD and ESRD require carefully coordinated care. Enrollment in diabetes classes (when appropriate), renal diet cooking classes, and support groups can be of tremendous benefit. By gradually introducing different educational materials and enabling the patient to help control the course of the disease, healthcare providers can help restore a sense of independence and confidence in the patient. It is crucial for healthcare professionals to realize that chronic illness is often a new and unanticipated event to the patient and family. Therefore, concrete information is vital. At the initial diagnosis, the family may be overwhelmed and struggling to come to terms with the illness. They may also be grappling to understand new medical jargon and trying to assimilate a tremendous amount of information in order to make decisions about medical care plans. At this juncture, enhancing communication between the primary physician or nephrology team and the patient/family is the primary goal . Technical information about the illness, prognosis, and care regimen should be conveyed. Healthcare professionals should be sensitive to the fact that this information may need to be relayed on several occasions. During this time, a list of resources and referrals may be helpful . Social isolation may occur. Therefore, it becomes a complicated issue to determine whether a particular behavioral change is the result of normal human development or illness- related. Technical information related to the daily care of the patient should also be relayed. Family members may have to be taught how to lift and move patients without hurting themselves or the patient and how to administer medications or dialysis . Family members should be reminded and educated about the physical consequences of the illness. Patients, for example, may experience fatigue as a result of the medications and/or the illness; however, some family members may become frustrated with the patient and interpret the patient as being lazy and taking advantage of the sick role .

Until now, you haven’t had many options. If you wanted to get lean, you had to diet strictly— and weeks of food deprivation stripped a little fat but also left you smaller and weaker.
If your goal was to get bigger, you had to eat like a pig. Then, of course, you’d pack on not only muscle but fat as well. The reason why both strategies lead to less than satisfying results can be answered in one word: carbs. Consuming large amounts of carbs (particularly the sugary and starchy kind) raises your blood sugar. This triggers the release of the hormone insulin to bring your blood sugar level back down. If you’ve just finished weight training, that’s good, because insulin will take the calories you’re consuming straight to the muscle cells for rebuilding. At any other time of day, however, insulin will store those calories as fat. Manipulating this effect is the key to getting the perfect body—lean, muscular, and strong. I’m going to outline two methods of carb manipulation I have researched, road tested, and ultimately trademarked: the Carb Nite system to lose fat and carb back- loading to pack on lean mass. You can alternate them throughout the year to stay big and lean simultaneously. While you’ll still have to choose whether you want to focus on losing fat or primarily gaining size, you won’t have to give up muscle or a trim waist to achieve either one. Here’s a fascinating paradox. Over the last 4 decades, nutrition policy makers have increasingly exhorted us to eat less saturated fat. As a result of this. New Weight-Loss Study: High-Carb Diet Beats High-Fat National Institutes of Health trial finds high-carb/low-fat better than high-fat/low-carb for losing body fat. My mouth is watering so much from getting the pictures together for this post that it doesn’t even matter that my water pipes are frozen right now. I’ve got water. You also won’t have to count calories or forsake your favorite foods. In other words, you have options—at last. As a former obese kid, I thought I’d never be able to stay muscular without being a little fat. Intermittent Fasting, or “IF”, is a relatively new craze that is used as a supplement to your diet. It revolves around the timing of your food intake, and can.Using these two strategies, I now maintain 6% body fat year- round without much effort and without giving up any of the junk food I love. Here’s how it works. Carb Nite If you want to get shredded and strong, use Carb Nite, which takes advantage of your body’s weekly hormonal rhythms to help you lose fat, maintain muscle, and increase strength. You can cut significant fat without even working out. Start With a 1. 0- Day Recalibration Prime your body to use fat for energy instead of carbs and stop all the processes that make it easy to store carbs as fat. You do this by following an ultra- low- carb diet for 1. Eat 3. 0 grams of carbs or fewer per day (approximately one piece of fruit or a small serving of oatmeal). Any starches and sweets in your meals must be extremely limited. Enjoy a Carb Nite On the evening of your 1. Your discipline can take a hiatus: eat pasta, pizza, french fries, or any other sugary/starchy carbs you can get your hands on. Want brownies or Krispy Kreme doughnuts? High- glycemic carb sources like these are actually better choices than sweet potatoes and rice. You need to refill your carb stores, crank up your metabolism, and give your mind a break. You’ll get the best results if Carb Nite falls on a day you lift weights, so try to time it accordingly. Don’t worry about getting fat. Several studies show that because of the change in enzyme production that occurs in your body throughout your low- carb days, gaining fat on a Carb Nite is nearly impossible. Lean Out At this point, you’ve gotten your body to switch over from using carbs to fat as fuel. Return to the menu you used during recalibration, but this time you won’t have to follow it as long. Eat 3. 0 grams of carbs per day and, once per week, have a Carb Nite. Note that this is a six- to eight- hour “night,” not a daylong carb binge. Maintenance This is where you get to look ripped all the time. Once you drop below 1. Carb Nites per week to keep your metabolism going and spare muscle mass. So you could have your first Carb Nite on Wednesday and your second that Saturday. One night per week, begin eating sugary/starchy carbs at 5 p. On training days, ingest carbs from post- training until bed. On non- training days, have a single carb meal at night. Postworkout Nutrition Muscles need carbs after a workout to replenish their energy stores and prevent further muscle breakdown. But if you’re on an ultra- low carb day as prescribed in the Carb Nite plan—or you’re doing carb backloading but must train early in the day—you can’t ingest many carbs without compromising the program. These supplements can help you get around this problem. We don’t recommend training without them. Carb Nite Consume 2. Carb Back- Loading The same as for Carb Nite, but add 3. Pick up Kiefer’s custom supplements, Blend D and Blend H (both whey and casein hydrolysates but mixed in different amounts) at dangerouslyhardcore. Pick up Kiefer’s custom supplements, Blend D and Blend H (both whey and casein hydrolysates but mixed in different amounts) at dangerouslyhardcore. What is Carb Back Loading? To lean out and gain muscle, try carb back- loading. As the name implies, this limits carb consumption to late in the day. Taking in carbs in the afternoon or evening is done for a strategic reason. Carbs make both muscle and fat cells grow—and often at the same time. But by shifting when you eat carbs, you can actually control which kind of tissue grows. As stated earlier, Carb Nite can be effective without training. Carb backloading, on the other hand, requires resistance exercise to work. Your body’s sensitivity to insulin is highest in the morning and lowest in the afternoon, leading many to believe that we should eat carbs first thing in the morning because much insulin won’t be required to keep blood sugar under control. The problem is that if you raise insulin even slightly by eating carbs—3. Worse, you may even get fatter because of the presence of another hormone—cortisol. A stress hormone, cortisol will break down fat all morning, but combined with raised insulin, it can actually cause your body to create new fat cells. For these reasons, most of your carb intake must come in the evening. Toss in a weight- training workout right before you eat carbs, and you maximize your ability for insulin to store them in your muscle cells while leaving fat alone. Studies in the Journal of Applied Physiology have demonstrated that lifting allows muscles to use and store sugar for several hours post- training—that means it will be quickly absorbed by the muscles you’ve trained to help them recover and grow. The best part? You get to eat tasty treats almost every day. Keep carbs at 3. 0 grams or fewer for five to six days and your body will store them more effectively. Start Gaining What and when you eat will depend on when in the day you train (and if it’s a training day or not). Afternoon/Evening Training This is the ideal setup. Until the after noon, keep your carbs low— 3. Begin your weight workout at some point between 3 p. Afterward, ingest a postworkout shake that’s rich in carbs, and keep eating carbs until you go to bed. The same foods prescribed on Carb Nite apply here—pizza, ice cream, and so on. It is not uncommon for followers of the plan to eat 4. Morning Training If you train in the morning, you’ll need to eat a small amount of carbs after your workout and take advantage of supplements that help spike insulin (see “Postworkout Nutrition” on the previous page) so that you can recover from your workout without throwing off the hormonal rhythms of back- loading. That night, around six, eat your carbs, but go mainly with less sugary sources like rice and potatoes. Non- Training Days On days you don’t lift (this includes days you just do cardio), limit carbs to a single late- day meal. Say, dinner or a dessert before bed. Training You don’t need to follow any particular regimen when employing either Carb Nite or carb back- loading. Since nutrition is the most important aspect of gaining muscle or getting lean, just make sure you fully commit to one eating strategy or the other. And make sure you follow “Postworkout Nutrition.”. Elevators of the Future Will Move Sideways Without a Single Cable. One of the biggest engineering challenges of building a towering skyscraper isn’t keeping the structure from falling over, it’s moving all the people around inside of it. To improve efficiency, and facilitate the construction of even taller buildings, Germany’s Thyssen. Krupp has completely redesigned elevators so that they can move sideways now, too. The traditional design of elevators has cars that only move up and down in a shaft, raised and lowered by a winch- powered cable. It works well enough in smaller buildings, but wait times become problematic as elevator shafts get taller and taller. That’s why the lobbies of larger skyscrapers have elevators that only service the upper floors. You. Tube’s Tom Scott recently visited Thyssen. Krupp’s 8. 00- foot tall testing tower in Germany, and there he experienced the company’s new MULTI elevator system that allows cars to move both vertically and horizontally using magnetic linear induction motors—the same technology that powers high- speed maglev trains. Eliminating the cable not only reduces the amount of infrastructure needed to run an elevator, it also allows shafts to be built almost infinitely long because the weight of the strong steel cable itself is no longer a limitation. But the biggest advantage is that elevators can now move sideways along horizontal shafts, allowing complete loops to be constructed inside a building, and elevators to hop between shafts in order to find the quickest route to their destination floor. To ensure the new system is as safe as the elevator design we’ve relied on for the past 1. Thyssen. Krupp is undertaking years of testing and refinement. But one day, it might eliminate the agonizing wait for the next elevator, delivering cars as frequently as every 1. Lose Weight With Us in 2016. Actually it’s not surprising at all, in previous articles (like this one, regarding the “eat less to weight less” myth) I’ve shown that there’s looooooads of research showing that overly- restricting your calories often leads to re- lapse. Freedhoff, talked about this in his article The Real Biggest Losers? The Show’s Audience . And while my personal opinions shouldn’t concern you, the peer- reviewed medical literature stemming from The Biggest Loser, as well as the AAP’s implicit endorsement of the show, should. He goes on to talk about how, due to the extreme caloric restriction, the metabolism of these participants had slowed down to an equivalent of 5. Side note: I talk about this more in the low- calorie myth article). What’s more? After interviewing three of the biggest losers he found that 8. But by far the best proof that this show does not have your best interests in mind comes from one of the executive producers on the show, JD Roth, saying: “For some of these people this is their last chance,” he said. It’s reality TV. It’s meant to be ridiculous for ratings because people need to be paid and feed their kids. It’s a lot like the news – there will be sensationalism to catch your attention. There are plenty of viewing dollars at stake. Here’s what worries me. And recently, children have been added into the mix. Eat 1/2 of what you do normally)Concentration camp amounts of exercise (the biggest losers were reportedly exercising 4- 6 hours a day)In reality, none of these are secrets, and neither of them are healthy long- term approaches to weight loss. I’m going to show you why you shouldn’t emulate these people, and what you should do instead.***Flaw #1 – Severe Caloric Restriction (= Starvation Diet)By far my biggest beef with this show is that the participants are just put on an absurdly calorie restricted diet. When the emphasis is simply to lose as much weight as possible, eating as little as possible will work in the short term (in the SHORT term), because the body is literally starving. Not- so- secret #1: dramatic caloric restriction. Here’s the problem: I’ve already talked about the very strong link between severe caloric restriction and re- bounding back (regaining the weight you lost). But it’s more than that – it’s reinforcing this ridiculous (and unscientific) weight loss mantra that you just “keep eating less to weigh less.”Okay, so I ate 5. Dear Harvard Health Publications Blog, Did my previous comment on the Paleo diet somehow violate your rules about comments that do not relate directly to the contents. Foods to Avoid. Before we dive into the enormous list of food that you can enjoy on a Paleo diet, here are the basics of what you should avoid. The Dude Diet: Clean(ish) Food for People Who Like to Eat Dirty . From chef and creator of the. Splenda *claims* you can use it 1:1 for sugar “in cooking and baking in a variety of recipes.” I guess I’ll just have to try it and let you know. A "vat" of this stuff lasts about a week, depending on how much you eat each day. Eat as much of The Cabbage Soup Diet as you like. When it comes to diet-friendly whipped toppings, there aren't many widely available choices. There's Cool Whip Free and Fat Free Reddi-wip. That's pretty much it. I’m not losing anymore. Drop it another 3. Okay, I’m not losing anymore, drop it by another 5. On and on this cycle goes until the person has no energy, loses half their muscle, and can barely function throughout the day all while wondering they aren’t losing weight anymore. What’s worse is that children are being brought on the show. So now, despite kids already being bombarded with images of ultra fit men and women, they’re subconsciously getting the idea that they need to just keep eating less to weigh less. Now we have eating disorders on the rise. Please. There are better, real, sustainable approaches to weight loss that don’t leave you praying for death.***Flaw #2 – Olympic Athlete Levels of Daily Exercise – 4- 6 Hours of Exercise (a DAY)Seriously? No wonder there is so much garbage diet information. No wonder people think that celebrities need to invest disgusting amounts of time in the gym. In one of my articles I profiled celebrities that had “gotten huge” for movie roles. And they pass the information onto their friends that also say “Yeah that sounds right. That’s a ton of exercise.”But let me set it straight here: no matter what your weight loss (or muscle gain) goal is – you will never need to exercise 4 hours a day. People on the Biggest Loser are putting in Olympic Athlete hours worth of exercise. Not only is this totally ridiculous, it’s totally unrealistic and unnecessary.***Flaw #3: Focusing Solely on Weight Loss. Every morning he was weighing himself on the scale, and it actually hadn’t budged much in three months. He was noticing maybe a 3- 5 pound difference in his weight, but it wasn’t anything dramatic. He needed to lose a lot more by his standards. When I asked him if he looked like he had lost weight, he said he did notice looking much different. So even though his belly had gotten smaller, the scale didn’t show it – and he was only measuring his progress by the scale. Don’t do that! I think everyone wants results quickly. It’s human nature. But health is a notoriously tough thing to change because results come slowly. So when you aren’t seeing results, just focus on improving. If you can only do 1 pushup now, try 2 next time. Believe me, your body will look much different when you can do 5. There’s this absurd idea that if you aren’t losing a minimum of 1. Let me be blunt here: if you weigh 4. But if you’re off your normal weight by 3. Lauryn that lost 1. Lamine lost 6. 6 pounds in something like 6 months. The cool thing? Neither one of them mentioned going hungry. They didn’t change the intake. It takes time, lots of it. It takes focus. It requires energy. And it’s probably going to entail more than a few failures. This is the long way of me saying this: if you are constantly focused on your arbitrary goal of “5. But if you invest into changing your habits, rather than relying on willpower, and if you find ways to enjoy the process and take it slow, you will inevitably reach your destination.***So What Should You do Instead? I understand that the Biggest Loser is supposed to serve some kind of morbid inspiration to a country that is sick and overweight. But it’s just plain terrible advice to emulate if you are looking for real, sustainable weight loss. Flat out I think it’s bad advice. Here’s what I think you should be doing instead: 1) Focus on eating the right food, and when you do, the right amount will take care of itself. I have profiled several success stories so far of dramatic weight loss – 1. Neither of the people counted calories and neither of them starved themselves. What they instead focused on was eating the right kinds of foods. Depending on where your current body weight is, just shifting to the right types of foods will naturally result in the right number of calories. I profiled this dramatically in this little experiment I did called . Besides the obvious no- fun of starving yourself, dramatically dropping your caloric intake is a great way to end up with stalled weight loss, decrease energy, poor sleep, and PMS (even if you’re a guy). Focus on eating real food. Ignore exercise (if you hate it) until you’ve built good food habits. Food should be the king of your weight loss pyramid. I’ve talked about it time and time again, but changing your eating habits will account for the majority of your success. Call that 6. 0%, 7. Personal trainers constantly complain that their clients train with them every week, but don’t change what they eat. Months pass, and they don’t see results. Extremely easy. If you hate exercise, just focus on dominating your food and getting better food habits. Check out my food control program, too. Set goals but ignore them – focus on progress. I’m a huge fan of ignoring goals.“What!? Preposterous! Aren’t goals the way to see results?”Here’s the thing: Goals are great to temporarily motivate you, but what happens when you get obsessed with the goal and ignore the other 3. You get miserable. You get tunnel vision. Your goal is “lose 5. You’re relying on the goal to make you happy. Forget that. That’s garbage. Your health is part of your life – and you need to be happy before you do anything. So here’s my suggestion to have fun, stay happy, and get results all without focusing on your goal: Just focus on progress. If you can only do one pushup, work on getting to 5. If you can’t do a pull- up, work on doing one strict pullup. If you can only run a 1/2 mile, work towards that 1 mile. If you drink soda every day, try working towards your first week soda- free. Health is inseparable from life. Try to stay sane and have fun as much as possible. The more you treat it like a beast to conquer, chances are the less fun it’s going to become. Thoughts on The Biggest Loser? What are your thoughts about this show? My biggest beef with it is really just two things: You don’t need to starve yourself or exercise 4 hours a day to get fit. Not even close. And one more thing: Just obsessing over the scale, to the exclusion of the many other factors that make you healthy and happy is a fool’s game. Being healthy is just as much internal as it is external, and you need to work on both to really feel whole. Leave a comment below with your thoughts on the biggest loser.
